Auto Endorsements
Enhance your auto protection with a range of endorsements for private passenger vehicles.
Policy change forms
Policy Change Forms, more commonly known as OPCFs, PCFs, or policy endorsements, are additional optional enhancements that allow you to make changes to your Ontario auto insurance policy to fit your needs. These are regulated by the Financial Services Regulatory Authority of Ontario (FSRA).
Your Ontario auto insurance policy may include change forms to enhance, add, or modify your coverage. OAP1 is the standard auto policy in Ontario. You can view it here. Check your Certificate of Insurance for those included in your policy.
You may have policy change forms included in your policy documents that require your signature. Please check your policy documents for these forms, sign them and return to your insurance broker as soon as possible.
Your certificate of automobile insurance includes the change forms that apply to you
The endorsements listed here are those that do not require your signature.
*Some endorsements may generate an additional premium charge.
AODA-compliant PDF versions are available upon request. Requests can be sent to gorecommunications@goremutual.ca.
